. � ' � S . <br /> AGREEMENT <br /> This AGREEMENT made and entered into this first day of <br /> May , 2000 between CITY OF DECATUR, ILLINOIS <br /> MUNICIPALITY (hereinafter referred to as "CITY" ) and ST. MARY ' S <br /> HOSPITAL, DECATUR, OF THE HOSPITAL SISTERS OF THE THIRD ORDER OF <br /> ST. FRANCIS, AN ILLINOIS NOT-FOR-PROFIT CORPORATION (hereinafter <br /> referred to as "HOSPITAL") , <br /> WITNESSETH : <br /> WHEREAS, the CITY is desirous of entering into a contract <br /> with the HOSPITAL which shall provide an Employee Assistance <br /> Program (hereinafter referred to as "EAP" ) through the Employee <br /> Service Center (hereinafter referred to as "ESC" ) , and <br /> WHEREAS, the HOSPITAL ' S ESC is desirous of providing <br /> assessment and referral services to employees and their <br /> dependents of CITY who are experiencing behavioral/health <br /> problems that have affected or could potentially impact their <br /> work performance, <br /> NOW THEREFORE, in consideration of the mutual covenants <br /> herein contained, the parties agree as follows : <br /> St. Mary's Employee Service Center, 1900 E.
