� ~ � � ` , � I •� ♦ , , � � i I <br /> � � f T � 1 ' , , I <br /> The proposed construction shall be performed at such a time as not to interfere with the <br /> State's mowing operations. <br /> All existing pavement gravel/crushed stone side roads and entrance drives disturbed by <br />� this construction shall be restored to original condition and cross section with same <br /> material as now exists, and shall be maintained in a smooth condition for passage of <br /> traffic at all times during construction. <br /> h II be restored to <br /> All existing bituminous surfaced areas disturbed by this construction s a <br /> original condition with a Grade CA-6 crushed stone (Grade CA or CA-10) base, <br /> surfaced with 2" of cold lay asphalt or bituminous concrete material, (Class I Surface) <br /> mechanically compacted in place. <br /> Any underground utility or drainage facility, whether farm tile, sanitary sewer, or storm <br /> sewer cut or damaged by this construction shall be restored to functional condition and <br /> to the satisfaction of the owner of the facility. Existing highway ditch drainage shall be <br /> maintained at all times. <br /> At all locations on the right-of-way where the proposed underground installation is laid <br /> closer than 5' from the near edge of a landscape tree, the utility shall be bored under the <br /> tree in such a manner that no part of the trench excavation shall be located closer than � <br /> 5' from the near edge of the trunk of the tree. <br /> All turf areas on State right-of-way disturbed by this construction shall be seeded or <br /> reseeded until a grass turf is reestablished. The disturbed areas shall be seeded with <br /> the specified mixture at the following ratio: Three pounds of Kentucky Bluegrass or <br /> Kentucky 31 or Alta Fescue, and two pounds of Perennial Ryegrass. The application <br /> rate shall be five pounds per 1,000 square feet. All areas shall be mulched with straw <br /> and areas with slopes greater that 3:1 shall be covered with an erosion control fabric. <br /> The Applicant shall locate, furnish, construct, and maintain the entire improvement at <br /> his expense and shall assume all risk or liability of every nature accruing during <br /> construction or thereafter as a result of the improvement. <br /> The Applicant shall furnish the contractor with one (1) copy of this permit. This copy <br /> shall be available for inspection at the job site by authorized personnel upon request. <br /> The Applicant shall not place or construct advertising signs on or overhanging State <br /> right-of-way. <br />
