The City encourages DBE participation for this Project. List all firms participating in this Project(your firm or any of your proposed sub-contractors) which qualify for DBE status, and the approximate, anticipated percentage of involvement. Place this item after Exhibit F, label it "Exhibit G", and title it "Participation of Disadvantaged Business Enterprises". (Each DBE company will be required to complete and submit to the City a DBE certification form with the final contract document.) j Exhibit H: Non-Collusion Aff davit- Complete and have notarized the form contained in Appendix B. Place this item after Exhibit G. Exhibit I.• "Debarred, Suspended or Ineligible Contractor" Certif cation of Eligibility- Complete and execute the form contained in Appendix B. Place this item after Exhibit H. Exhibit J.� References from Previous Clients - Respondents must furnish letters of reference from at least five (5) of the clients listed in Exhibit E. Place these letters behind a cover sheet labeled "Exhibit J" and titled "References from Previous Clients", and then place the entire exhibit behind Exhibit I. Exhibit K: "Restrictions on LobbyinQ" Certif cation - Under United States Public Law 101-121, Section 319, titled"Restrictions on Lobbying", if any resultant contract involving Federal funds exceeds One Hundred Thousand Dollars ($100,000.00),the Proposer will be required to submit a completed, fully executed copy of the "Restrictions on Lobbying" Certification form enclosed in Appendix B. CompilinQ the Proposal Document- When you have completed the preparation of all the requested information and forms, create a document cover and a"Table of Contents"to place in front of the other materials. Page#1 of the Proposal will begin with the first page of Exhibit A, after the document cover, the "Table of Contents" and the Cover Letter. Right of Selection/Rejection - The City reserves the right to select or reject any or all proposals far any reason, to waive any informalities in any proposal received, and to waive minor deviations from these instructions. However, Proposers who submit proposals which do not follow the instructions, or do not provide the information requested within this RFP, may be subject to immediate rejection. BudQet- The City has budgeted $937,500 for this Project, exclusive of land acquisition costs. That amount of money will be for architectural and engineering services, demolition of the existing parking garage, construction of the new facility and site development.
