Exhibit A: Review of the Project- Explain your company's understanding of this Project in terms of the Project's components and the City's objectives. Identify those architectural, engineering and transportation design issues which you feel will be necessary to address during the course of this Project. Further, explain how your firm/team has addressed these issues in previous consulting assignments. Place this item after the Cover Letter, label it "Exhibit A", and title it "Review of the Project". Exhibit B: Proposed Scope of Work- Based upon your findings in Exhibit A, delineate each work task required to successfully complete this Project. For each work task, note when decisions will be required from the City. Place this item after Exhibit A, label it "Exhibit B", and title it "Proposed Scope of Work". Exhibit C: Proposed Schedule of Work- Prepare a work schedule graphic illustration which lists all work tasks in E�chibit B. Since the schedule of work is greatly dependent on the completion of work by other contractors,the proposed work schedule should reference periods of time before, or after, work to be done by other contractors. Designate when decisions will be required from the City. Place this item after Exhibit B, label it"Exhibit C", and title it "Proposed Schedule of Work". Exhibit D: List of Sub-contractors - List all companies participating in this Project, and describe in detail each company's specific function(s) in relation to this Project. Place this item after Exhibit C, label it "Exhibit D", and title it"List of Sub-Contractors". Exhibit E: Summary of Experience with Similar Projects - Submit a list of constructed projects similar in nature to the proposed Project in which your firm, or any of the proposed sub-contractors, was employed as the owner's representative. The following information must be included for each project: a)name of the project; b) the project owner; c) the project completion date; d)the total construction cost; e)the project components similar in nature to this Project(i.e. local transit and intercity bus terminal; rail station; etc.); and � a contact person of the owner who is familiar with the project (include this person's telephone number). Place this item after Exhibit D, label it"Exhibit E", and title it"Summary of Experience with Similar Projects". Exhibit F: List ofKev Personnel- Provide a list of your firm's, and each proposed sub-consultant's, key personnel who will be assigned to this Project. Include a resume for each person and explain in detail each key person's role in this Project. Place this item after Exhibit E, label it"Exhibit F", and title it "List of Key Personnel". Exhibit G: Participation of DisadvantaQed Business Enterprises - It is the policy of the City to support and encourage participation of Disadvantaged Business Enterprises (DBEs) in the utilization of programs, activities, and contracting opportunities funded in whole or in part by the United States Department of Transportation in accordance with the regulations contained in 49 CFR Part 23, as amended. DBEs consist of small business concerns which are at least fifty-one percent (51%) owned and controlled by one or more socially and economically disadvantaged individuals.
