.� - 5. The Cab Company shall provide adequate insurance and shall assume all liability associated with the provision of its services for the Program. 6. Cab Company drivers shall assist in the loading and unloading of disabled passengers, as well as their pazcels and personal effects. 7. The Cab Company will avoid any undue delay, either at point of pickup or enroute and shall strive to pick up Program patrons within 5 minutes of the scheduled time. , 8. Cab Company vehicles shall not•wait for Program riders for more than five (5)minutes at any point. 9. The Cab Company shall account for all patrons transported together with the appropriate charge and shall maintain accurate trip sheets, cab ride forms, and other data which will provide sufficient information to permit evaluation of the Program by the City and the Federal and State funding agencies. 10. The Cab Company shall pernut the City to review or audit all records pertaining to this Program. 11. The Cab Company agrees that this agreement is subject to the provisions of E�chibits A, B and C hereto attached and made a part hereof. C. Reimbursement for R.ides: 1. The fares charged Program participants by the Cab Company shall be made on the basis of existing rates ($1.50 initial charge plus ten cents (.10) for each one-tenth(1/10) mile, fifry cents (.50)for each additional passenger, and twenty cents (.20)per minute waiting time after 15 minutes.) 2. The Cab Company shall bill the City at the end of each month for the cab fares charged for providing transportation services to Program participants. 3. Any change or alteration in the taxi-cab fare schedule shall require prior concurrence of the City. 4. The Cab Company shall submit a voucher for payment to the City at the end of each month. 5. Copies of all cab ride forms and trip sheets shall be submitted to the City at the same time as the voucher for payment. � - � -
