A "client contact", for purposes of <br /> information and assistance means the receipt of a request and the response <br /> thereto; each request plus response equals one client contact. <br /> Outreach Service - a service whereby the service provider initiates <br /> contact with a senior citizen for the purpose of providing such citizens with <br /> information regarding various substantive services available to the citizen. <br /> Such contact must be in person, and one-to-one. Available substantive <br /> services are the same as those included under Information and Assistance. A <br /> "client contact", for the purposes of outreach service, includes any direct <br /> personal contact with a senior citizen, in the home of the senior citizen or <br /> family member thereof, following the initial contact, if made for the purpose <br /> of providing additiorial information or follow-up to information already <br /> provided. <br /> Circuit Breaker Program - a state-wide property tax relief program <br /> administered by the Illinois Department of Revenue which issues cash grants <br /> to Illinois residents 65 years of age or older who have household incomes <br /> under a certain level. Applications must be filed with the State on or before <br /> December 31. The service provider would assist such persons with these <br /> applications and file them for the citizen, and provide follow-up services. <br /> Pharmaceutical Assistance Program - a state-wide program for persons <br /> who are eligible for the Circuit Breaker program whereby the State of Illinois <br /> will reimburse qualified persons for part of the costs for certain medications. <br /> The service provider would assist such person with the applications for this <br /> program and file them for the citizen, and provide follow-up services. <br /> � COLDFACTS Program - a county-wide medical information program <br /> whereby medical information regarding a senior citizen is recorded on a form, <br /> inserted in a bottle and placed in his or her refrigerator for use by medical <br /> emergency response teams. The service provider would assist in the <br /> completion of the medical information form and verify that it is correctly <br /> placed in the refrigerator, and update regularly. <br /> Carrier Watch Program - a city-wide home check safety program <br /> whereby a senior citizen attaches a sticker to his or her mailbox, after <br /> receiving such sticker through an application process. Mail which is not <br /> retrieved from the mailbox by the next delivery time will cause the Postal <br /> Service to notify the service provider. The service provider would assist in <br /> the completion of the necessary forms and issue the appropriate stickers, and <br /> update regularly. <br />
