2. FEE. The City shall pay the Center Seventy-One Thousand Dollars ($71,000) for the <br /> performance of those functions and services hereinbelow described. Such sum shall be paid at a <br /> rate not to exceed Five Thousand Nine Hundred Sixteen Dollars and Seventy-Four Cents <br /> ($5,916.74)monthly during the term hereof, and payment shall be subject to full compliance with <br /> all performance standards established herein, subject also to the provisions of paragraph 5. <br /> 3. FLTNCTIONS AND SERVICES. <br /> A. The Center shall perform the following functions and services, and shall meet <br /> the minimum standards provided hereby: <br /> SERVICE SERVICE STANDARD <br /> Information and Assistance 2,200 client contacts annually <br /> Outreach Service 1,500 client contacts annually <br /> Circuit Breaker Program 900 applications completed annually <br /> Pharmaceutical Assistance Program 200 applications completed annually <br /> COLDFACTS Program 700 applications completed/updated annually <br /> Carrier Watch/Telephone Reassurance Program 150 applications completed/updated annually <br /> All senior citizens assisted for the purpose of this paragraph shall be residents of the City <br /> of Decatur. <br /> B. The Center sha11 publish and distribute, as described herein, a bi-monthly newsletter <br /> and a directory of services. The bi-monthly newsletter shall discuss and describe the services <br /> available from the Center, provide dates for upcoming events and program deadlines, and <br /> generally keep readers apprised of local matters relevant to senior citizens.
