services shall contain names, addresses, telephone numbers and brief descriptions of agencies <br /> and organizations providing services to senior citizens, in a manner substantially similar to the <br /> directory of same published and distributed by the City's Office on Aging as of April, 1995. The <br /> newsletter and the directory shall be provided free of charge to senior citizens who are residents <br /> of the City of Decatur, the newsletter to not less than 8,000 persons per issue, on average, and the <br /> directory of services to not less than 6,000 persons per year. <br /> 4. REPORTS. <br /> A. The Center shall furnish monthly reports to the City's Departrrient of Community <br /> Development, in a form satisfactory to the Director of said Department, detailing the Center's <br /> compliance with the service standards established hereby, and such other information as such <br /> Director deems material and relevant to the purposes hereof, both for the applicable month and <br /> for the year-to-date. At the same time such reports are furnished as herein required, the Center <br /> shall provide minutes of its last preceding regular monthly board of directors meeting. The City <br /> shall not be obligated to pay any fees to the Center for any month until the City has received such <br /> report and minutes, subject to the provisions of paragraph 5. <br /> B. Within thirty (30) days of the beginning of the term of this Agreement, the Center <br /> shall provide the City with the following: <br /> (1) a mission statement adopted by the board of directors of the Center expressing <br /> commitrnent to the goals and objectives of this Agreement; <br /> (2) a list of the names and addresses of the members of said board. <br /> C. Such reports and other information as are required hereby may be audited by the City <br /> at the discretion of the Director of Community Development, and the Center shall cooperate fully <br /> in such effort. <br />
