and liabilities incurred in or about any such claim for bodily injury or death, or any action <br /> or proceeding brought herein. <br /> 10. Insurance Requirements. Lessee shall obtain, at its expense, and keep <br /> in force during the term of this Lease, comprehensive general liability and property <br /> damage insurance in an amount of not less than $1,000,000 single limit, $2,000,000 <br /> aggregate, for bodily injury, death and property damage to any one person and subject to <br /> the same limitations far any one occurrence. The comprehensive general liability policy <br /> shall include coverages for premises and operations, a blanket contractual liability <br /> coverage which includes the indemnification agreement contained in the next paragraph, <br /> broad form property damage, independent contractors and personal injury liability <br /> coverage. The policy issued for the afore-described coverages shall name the City of <br /> Decatur, Illinois, as an additional insured. Such insurance shall not be canceled prior to <br /> termination of this Lease. <br /> 11. Certificate of Insurance. Prior to the commencement of the terms of <br /> the Lease, Lessee shall file with the City a Certificate of Insurance, in a form acceptable <br /> to the City, signed by an authorized agent of the insurance company or companies writing <br /> the insurance described in said Certificate showing complete coverage of all insurance <br /> required in this Lease. Such Certificate shall certify to the following: Name and address <br /> of party insured; name and address of authorized agent executing such Certificate; the <br />, description of the type of insurance, and the coverages afforded thereunder, the insurance <br /> policy numbers; the limits of liability of such policies and the date of expiration of such <br /> policies. It is an affirmative obligation upon the Lessee to advise Assistant City <br /> Mana er Bill T s at the Ci of Decatur, 1 Ga K. Anderson Plaza, Decatur, IL 62523 <br /> g Y Yu �Y rY <br /> within two days of the cancellation or substantive change of any insurance policy set out <br /> above, and failure to do so shall be construed to be a breach of the lease. The Certificate <br /> shall specifically state that(1) the insurance certified therein contains contractual liability <br /> coverage for the indemnity agreement contained in this agreement, subject to the terms, <br /> conditions and exclusions of the policy, and (2) the City of Decatur, Illinois is named as <br /> an additional insured on the comprehensive general liability and property damage <br /> insurance described above. The name and address of the Certificate holder shall be stated <br /> as follows: The City of Decatur, Illinois, a municipal corporation, One Gary K. Anderson <br />
