LEASE AGREEMEN'T <br /> 1310 North Summit Ave. <br /> This Lease Agreement is made and entered into this December 29,2015,by and between A <br /> Step Forward Inc., hereinafter referred to as "Lessee" and Michael Stokowski, Wendy <br /> Stokoti�Tski hereinafter referred to as"Lessor"; <br /> WITNESSET'H: <br /> 1. This Lease Agreement shall become effective on the 29th day of December,2015. <br /> 2. Lessor, for and in consideration of the rents herein reserved to be paid by Lessee, <br /> does hereby lease to Lessee those certain premises commonly known as 1310 North Summit <br /> Ave, Decatur; IL, and more specifically described on Exhibit "A" attached hereto and <br /> incorporated herein by this reference, together with all easements, rights and appurtenances <br /> pertaining thereto and with the building and other improvements erected upon the demised <br /> premises and the person property located therein, which personal property is described on <br /> Exhibit"B"attached hereto. <br /> 3. The term of this lease shall be for a period of 60 months commencing on the Effective <br /> Date. <br /> 4. Rent shall be paid monthly in the sum of$ $ 600.OQ above principal and interest, in <br /> advance without any prior demand therefor at Lessor's address in Monticello, Tllinais <br /> commencing on the Effective Date, and thereafter on or before the 1 St day of each and <br /> every month during the term of this Lease. <br /> 5. In addition to the payments reserved herein to Lessor as rent, Lessee shall pay the <br /> following: <br /> a) all fees for occupational licenses and any other licenses or permits <br /> necessary in the operation of the business to be carried on in the demised premises; <br /> b) all charges far utility services provided to the demised premise, <br /> including, but not limited to, water, gas, electric, sewer,trash hauling,maintenance or <br /> service fees and telephone charges, as they fram tinie to time shall accrue and shail be <br /> due and payable; and <br /> c) any ad valorem real estate taxes and general and special <br /> assessmen#s against the demised premises incurred solely by reason of any further <br /> EXHIBIT <br /> � �� <br />
