AMENDED A(aREEMEN'T'TO DELEGAT'E t�UTHORIT'� <br /> TO gSSbJE PERNTI'TS�OIt WATER 1VI�1IN EX'TEI�TSIOIV <br /> Yn 1980, the General Assembly amended the Environmental Protection Act to allow ghe Illinois Env�ronmental <br /> Protection Agency(Agency)to enter into agreements to delegate to units of local government authoraYy to issue <br /> construction and operating permits for sewer and water main extensions. In response to this amendrnenY,the Agency <br /> promulgated rules for delegation(35 Ill.Adm. Code 174). <br /> On July 1, 1994 the City of Decatur(City)and the Agency entered into an amended agreement to delegat�authority <br /> to issue permits for water main extensions. �y letter, dated December 29, 1994, the CflY}� reques�ed that the <br /> Agreement be amended. The Agency has reviewed this rec�uest and found it to be proper, <br /> '�'herefore,pursuant to 415 IL,CS 5/4(g)(1992)[�il.Itev. SY�t. �981,ch. 111%z,par. 1004(g)] and 35 �1�.Adm. Cod� <br /> 17�,the Agency and the City enter ento�Yae following agreernent. 'I'he A�ency vvilY dele�at� its authority to issue <br /> construction and operating permits for wa�er main exYeensions of the City's distribution syster�a in return for the City°s <br /> agreement to administer the permit program according to the following conditions: <br /> 1. Stephen P. Swanson,P.E.,062-050185,shall be in responsible charge of the review and preparation of plan <br /> documents. In Mr. Swanson's absence,Matthew C.Newell,P.E.,062-048941,shall be in responsible charge <br /> of the review and preparation of plan documents. <br /> 2. Bruce A. McNabb, P.E., 062-048695, shall be in responsible charge of issuing construction or operating <br /> permits. IVo permit shall be valid unless it is signed by Mr.McNabb. 1VIr. Mc1�labb shall not�articipate xn <br /> the revievv or preparation of plarb documents. <br /> 3. T'he water main specifications by which plans shall be reviewed are the American iNater Works Associatiorn <br /> Standards for materials and the °'City of Decatur, Illinois Standard Specifications for Water Main <br /> Construction"Revised October, 1989. <br /> 4. Documents,plans,analyses and permits shall be conelated by assigning a City Engineering Division filing <br /> system number(which is a sequential number followed by the City's current fiscal year)at the time of project <br /> initiation and carried with the proJect to completion. <br /> Se '�'he City shall perform the fol➢owing routine operations: <br /> a. �Iater sampYes shall be collec�ed from the completed main ir► acco�dance �vitla'g'echnlca� �olicy <br /> Statement 652.203 (35 �19.Adm.�ode 652.203). <br /> b. If there aze changes in construction from the preliminary plans described in annual reports, the <br /> Agency shall be notified of those changes within 10 days after construction has begun. <br /> c, If the construction shovvr� in the as-built plans �iled in the annual report does not conform to the <br /> approved specifications or 35 Ill.Adm.Code Subtitle F,Chapter I and II,corrections shall be made <br /> �onsistent vvith ghose re�ulationse <br />
