Plans prepared for each water main extension project shall bear the professional seaY of the person <br /> as described in 35 Ill.Adm. �ode 174302. <br /> c. Trustees of a land trust who apply for a permit relating to the land which is the subjec�of the t�sY, <br /> must identify and define the interest of each beneficiary as required by °'� AC'T go require <br /> disclosure,under certification of perJury, of all beneficial interests in real property he�d in a land <br /> trust in certain cases,°'(765 ILCS 405/2(1992) [formerly Ill.Rev. All necessary authorizations from I1�0'I', Division of iWater l�esources and�J,S, Aa°my Cmrps of <br /> Engineers must be received for water main extensions which cross streams and draina�e ways. <br /> 7. Operating permits may be issued only after the following conditions have been met: <br /> a. The newly installed water main is flushed to remove foreign material introduced into the water main <br /> during construction and chlorinated for disinfection purposes; <br /> b. Bacteriological analyses are �erformed on a sufficient number of representatrve dvater samples <br /> collected frorg►ghe newYy constavcted water main to demonstrate satisfactory disinfect�on; and <br /> c, T'he vvater main is not placed into operation until such time as the bacteriological ana➢yses required <br /> in Subsection(b)indicate that the water is safe for drinking. <br /> 8. The following reports shall be submitted by the City to the Agency's Division of Public Water Supplies by <br /> February 1 of each year this agreement is in effect: <br /> a. Preliminary plans which indicate the location of anticipated water main construction for the ensuing <br /> 12 months. This may be updated to add or delete anticipated extensions by the public water sup�ly <br /> at any time; <br /> b. I'lan drawin�s as described in T'echnical Policy Statement 652(A),(�)and(C)�35 Ill.AdYn.Coc�e <br /> 652.102-108;652,Subpark�;and 652,Subpart C)for all water main extensions consYructed in the <br /> preceding year along with the identification numbers, dates of anitial operation, �eports of <br /> bacteriological analyses, length and size of water main,and estimated population served by�he�ew <br /> main; <br /> c. I�eports of bacteriologic�l analyses performed according to 35 Ill.Adm.Code 174.309(a);and <br /> d, A llst of constriaction and o�erating�ermits issued. <br />
