� � �• . �'� .-. <br /> • ` Agreement for Inspection Services <br /> . <br /> City of Decatur <br /> Lake Decatur Spillway Rehabilitation <br /> June 7,1995 <br /> This is an agreement between the City of Decatur (Owner) and H.L. Chastain, &Associates, <br /> LLP, (HLC). This agreement is necessary since the Owner is seeking to provide on-site <br /> representation to enhance the quality control of the project. The project is the construction of <br /> the Lake Decatur Spillway Rehabilitation. <br /> HLC is hereby proposing to provide the on-site representation as sought by the Owner. The <br /> on-site representative site visits will average four (4) days per week at eight (8) hours per day. <br /> It is estimated that there are 25 weeks remaining in the construction period. This would bring <br /> the estimated total time for ins ection services to 800 hours. It should be understood that <br /> P <br /> there may be times when the construction schedule requires full-time inspection as well as <br /> times when the need could be less. <br /> The compensation for the on-site project representative(s), direct expenses, office personnel <br /> and mileage cost will be charged on an hourly/unit cost basis in accordance with Exhibit A of <br /> the Agreement. <br /> For H.L.
