For home health aide services, the approved pian of care will be <br /> _ developed in consultation with appropriate staff of the certi�ied home <br /> health agency. <br /> Home heaith aides are required to report to the demonstrarion site any <br /> observed changes in the participant's condition or family situation which <br /> mav affect his/her currently approved plan of care. <br /> Health and personal care tasks which may be performed by a home heaIth <br /> aide include but are not limited to: <br /> a. dressing and wound care, including care of decubitus and stasis <br /> ulcers; � <br /> b. routine catheter care, including irrigation; � <br /> c. assistance with medications specifically orde:ed by a physician which <br /> are ordinarily self-administered; <br /> d. assistance with or supervision of tasks associated with activities of <br /> daily living, e.g.,bathing, toileting, grooming and dressing, transferring <br /> and use of adaptive equipment, where indicated; � <br /> e. training participants and primary careEivers in necessary self-help <br /> slfllls. <br /> While the primary functions of a home health aide are to provide health <br /> and personal care services, it may be deemed efficient under certain <br /> conditions, as reflected in the approved plan of care, that certain assigned <br /> household activities be performed in conjuncrion with a home health aide <br /> visit These ancillary services may include: <br /> a. pe:forming Ii�ht cleaning tasks in areas of the home used by the <br /> participant, e.g., dusting, vacuuming, damp mopping; <br /> b. assisting with or supervising the preparation of a shopping list and <br /> preparing or assisting in the preparation of ineals appropriate to a <br /> participant's dietary needs and financial circumstances; <br /> III-5 <br />
