9. To submit, upon request by the Public Agency a list of the personnel and the equipment he proposes to use in fulfilling the requirements of this agreement. 10. During the performance of work under this agreement the Engineer, for himself, his assignees and successors in interest agrees to conform to the provisions of Exhibit A hereto attached and made a part hereof. 2. The ENGINEER shall procure and maintain, at its expense, insurance of the types and in the minimum amounts stated below: a. Workman's Compensation Insurance in full compliance with the Workman's Compensation Act and Employers Liability coverage of not less than as defined by Illinois State Statutes. b. Comprehensive General Liability Bodily Injury $250,000/500,000 each occurrence Property Damage $250,000 each occurrence or $500,000 Single Limit Liability c. Comprehensive Automobile Liability Bodily Injury $250,000 single per occurrence Including Death $500,000 two or more, per occurrence Property Damage $100,000 each occurrence d. Architects & Engineers Professional Liability $250,000 e. Umbrella over above 2nd & 3rd items $1 ,000,000 The Comprehensive General Liability policy shall include blanket contractual liability coverage or a contractual liability endorse- ment covering the liability assumed by the ENGINEER under this AGREEMENT, with limits not less than those specified herein. Certificates furnished hereunder shall reflect such coverage. Insurance shall be written by a company or companies satisfactory to the OWNER. Before commencing any work hereunder, a certificate evidencing that maintenance of said insurance shall be furnished to the OWNER, and shall contain the following statement: � "Insurance evidenced by this certificate will not be canceled or altered unless 30 days written notice is given to the certificate holder." Policies for ITems 2 & 3 above must name the OWNER as an additional insured during the life of this AGREEMENT.
