3 . To enter each sewer line, where possible, with a field <br /> crew to make spot inspections in order to substantiate <br /> the information on the video tapes. <br /> 4 . To prepare a written report which will indicate recom- <br /> mended rehabilitation of each sewer line including cost <br /> estimates and prioritizing thereof. <br /> 5. To deliver ten (10) copies of the report to the City of <br /> Decatur one hundred twenty (120) days after receipt of a <br /> written Notice to Proceed from the OWNER. <br /> 6. The ENGINEER shall procure and maintain, at its expense, <br /> insurance of the types and in the minimum amounts stated <br /> below: <br /> (1) Workman's Compensation Insurance in full compliance <br /> with the Workman's Compensation Act and Employers <br /> Liability coverage of not less than as defined by <br /> Illinois State Statutes. <br /> (2) Comprehensive General Liability <br /> Bodily Injury, $250, 000/500, 000 each occurrence <br /> Property Damage, $250, 000 each occurrence or <br /> $500, 000 Single Limit Liability <br /> (3) Comprehensive Automobile Liability <br /> Bodily Injury, $250, 000 single per occurrence <br /> Including Death, $500, 000 two or more, per <br /> occurrence <br /> Property Damage $100,000 each occurrence <br /> (4) Architects & Engineers <br /> Professional Liability $1, 000, 000 <br /> BGM 90053A -3- <br />
