, , � Page 5 <br /> INSTRUCTIONS TO BIDDERS <br /> QUALIFICATIONS <br /> Any firm submitting a proposal in response to this Request <br /> should provide a full description of their experience in provid- <br /> ing the services enumerated in the Section entitled "Administra- <br /> tion of Self-Insurance Program." <br /> REFERENCES <br /> Each bidder should submit a list of at least 5 references <br /> from clients for whom they have provided similar services as <br /> those specified in this Request. <br /> CLAIMS ADMINISTRATION <br /> A detailed explanation of how claims will be handled should <br /> be included in the proposal. Give the name and address of the <br /> claims handling organization(s) , if other than the firm submit- <br /> ting the proposal. The company selected must agree to permit <br /> review of claim files by an independent, professional claims <br /> auditing firm or representatives of the City's funding <br /> agencies--the U.S. Department of Transportation and the Illinois <br /> Department of Transportation. <br /> SPECIMEN REPORT <br /> A sample copy of a monthly loss experience report should be <br /> included with the proposal. <br /> DURATION <br /> The contract for administrative services for the <br /> self-insurance program for the transit system shall be a three <br /> year agreement. However, either party to the agreement shall <br /> have the right to terminate said agreement if notice is given 90 <br /> days prior to any anniversary date. <br />
