ATTACHMENT "A" <br /> 1 . House Bill 3857 was passed in response to Public Act 85-1250 <br /> and this bill created the Community Integrated Living <br /> Arrangement (CILA) Licensure & Certification Act to be <br /> administered by the Illinois Department of Mental Health and <br /> Developmental Disabilities (DMHDD) . This Act provides for <br /> licensure of community aqencies ta provide Community <br /> Integrated Living Arrangement Services for persons with mental <br /> illness or developmental disabilities. CILA Services are <br /> intended to promote independence in daily livinq & economic <br /> self-sufficiency. CILA means a livfnq arranqement and support <br /> services certified by an Agency under this Act where eight (8 ) <br /> or fewer individuals with developmental disabilities or mental <br /> illness reside ( individually or as a qroup) under the <br /> supervision of an Agency. <br /> 2. In conjunction with the CiLA concept, in December 1989, the <br /> DMHDD awarded funding contracts to community agencies, <br /> includinq Macon Resources, Inc. , to develop an array of <br /> services, including residential for adults with developmental <br /> disabilities inappropriately placed in Nursing Homes. <br /> 3. The home at 4820 E. Cantrell will be leased jointly by four <br /> ( 4 ) individuals who either currently reside in a Nursing Home <br /> or are at risk of placement in a Nursing Home. <br /> 4. Utilizinq CILA resources, Macon Resources, Inc. will provide <br /> staff coverage 24 hours per day to support these individuals <br /> in the home; Day Program services outside of the home as <br /> requested by the individuals; and Case Coordination services. <br /> 5. DMHDD is responsible for providing Macon Resources, Inc. with <br /> onqoinq fundinq for all support services. <br /> 6. The four ( 4) tenants will be responsible far paying all ' <br /> household expenses. <br /> 7. This home at 4820 E. Cantrell should very nicely meet the <br /> needs of the tenants with a few minor repairs, some <br /> redecorating, and minor renovations to make the home <br /> wheelchair accessible. <br /> S. At this time the home meets all the State and Local <br /> regulations f�r this type of CILA Iiving arrangement. <br />
