transportation of such persons from hospitals or other <br /> places within the City to places in locales where such <br /> ambulance services are based outside the Cit or does <br /> Y, <br /> it include transportation of handicapped persons under <br /> programs operated by or on behalf of the Cit . <br /> Y <br /> 4. Applications for a license to operate an am- <br /> bulance and renewal thereof shall be made upon forms <br /> prescribed by the City Manager. The application shall <br /> contain: <br /> a. The name and address of the applicant; <br /> � b. The trade or other fictitious name, if any, <br /> under which the applicant does business or <br /> propose to do business; <br /> c. The location and description of the place or <br /> places from which the ambulance service oper- <br /> ates or is intended to be operated; <br /> d. A description pf each ambulance which the <br /> applicant operates or intends to operate <br /> within the license year, including the make, <br /> model, year of manufacture, serial number, � <br /> and the classification thereof under the Act. I <br /> e. Such other information as the City Manager may I <br /> reasonably require in the administration of <br /> the provisions of this Chapter. <br /> 5.
