PRIVACY NOTICE In the course of providing cable television service, it is necessary to maintain certain information regarding our customers. Under the federal cable television law, 17 U.S.C. §601 et seq,, we are required to provide this notice to advise our subscribers of the limitations under the law upon the collection and disclosure of such personal information.We collect the following types of personally identifiable infor- mation regarding our subscribers: 1. Name 2. Address 3. Telephone numbers (home and work) � � 4. Social security number and driver's license number 5. Billing records 6. Levels of service subscribed to 7. Maintenance and repair records 8. Converters and other cable equipment on subscriber's premises (if any) We use such personal information regarding our subscribers only for legitimate business ac- tivities related to the provision of services over our cable system. Without appropriate written or elec- tronic consent from the affected subscriber, we can not use the cable system to collect personal infor- mation unless it is necessary to render a service to the subscriber in connection with the cable system, or to detect unauthorized cable reception. Unless we obtain appropriate written or electronic consent from the affected subscriber, we can disclose such personal information only whenever it is needed for legitimate cable business purposes, or if such disclosure is required by court order. If we are served with such a court order, the affected subscriber must be notified before disclosure can be made. Under the cable law, cable subscribe�s must be afforded an opportunity to appear and contest the disclosure of such iriformation to a govern- mental entity pursuant to a court order. A governmental entity may obtain a court order for such infor- mation only if it offers clear and convincing evidence that the subject of the information is reasonably suspected of engaging in criminal acti��ity and that the information sought would be material evidence in the case. Under the cable law, we are also allowed to disclose subscribers' names and addresses for pur- poses which are not related to the provision of services over the cable system. However, if you would prefer that your name not be so disclosed, you may fill out a non-disclosure form which you can obtain from our local business office and return it to us. When we disclose names and addresses for any such purpose, we are not allowed to disclose the extent of viewing or any other uses made of the services we provide, nor can we disclose the nature of particular transactfons made using the cable system. The types of persons to whom personal information may be disclosed in the course of our cable business include our employees, agents, affiliates, sales representatives, accountants, billing and col- lection services, programming suppliers, and authorized representatives of the franchising authority. Personal information will be maintained by the cable system only so long as it is necessary for legitimate cable business purposes. Thereafter, such information will be destroyed unless there are pending requests for access to such information either by the affected subscriber or pursuant to a court order. Subscribers may review any such personal information which pertains to them and may request correction of any errors in such information. If you should desire to review such information at our local business office, please notify us in writing and an appointment will be arranged promplty for such in- spection during regular business hours.
