PRIVACY NOTICE <br /> In the course of providing cable television service, it is necessary to maintain certain information <br /> regarding our customers. Under the federal cable television law, 17 U.S.C. §601 et seq,, we are required <br /> to provide this notice to advise our subscribers of the limitations under the law upon the collection and <br /> disclosure of such personal information.We collect the following types of personally identifiable infor- <br /> mation regarding our subscribers: <br /> 1. Name <br /> 2. Address <br /> 3. Telephone numbers (home and work) � <br /> � 4. Social security number and driver's license number <br /> 5. Billing records <br /> 6. Levels of service subscribed to <br /> 7. Maintenance and repair records <br /> 8. Converters and other cable equipment on <br /> subscriber's premises (if any) <br /> We use such personal information regarding our subscribers only for legitimate business ac- <br /> tivities related to the provision of services over our cable system. Without appropriate written or elec- <br /> tronic consent from the affected subscriber, we can not use the cable system to collect personal infor- <br /> mation unless it is necessary to render a service to the subscriber in connection with the cable system, <br /> or to detect unauthorized cable reception. <br /> Unless we obtain appropriate written or electronic consent from the affected subscriber, we can <br /> disclose such personal information only whenever it is needed for legitimate cable business purposes, <br /> or if such disclosure is required by court order. If we are served with such a court order, the affected <br /> subscriber must be notified before disclosure can be made. Under the cable law, cable subscribe�s <br /> must be afforded an opportunity to appear and contest the disclosure of such iriformation to a govern- <br /> mental entity pursuant to a court order. A governmental entity may obtain a court order for such infor- <br /> mation only if it offers clear and convincing evidence that the subject of the information is reasonably <br /> suspected of engaging in criminal acti��ity and that the information sought would be material evidence <br /> in the case. <br /> Under the cable law, we are also allowed to disclose subscribers' names and addresses for pur- <br /> poses which are not related to the provision of services over the cable system. However, if you would <br /> prefer that your name not be so disclosed, you may fill out a non-disclosure form which you can obtain <br /> from our local business office and return it to us. When we disclose names and addresses for any such <br /> purpose, we are not allowed to disclose the extent of viewing or any other uses made of the services we <br /> provide, nor can we disclose the nature of particular transactfons made using the cable system. <br /> The types of persons to whom personal information may be disclosed in the course of our cable <br /> business include our employees, agents, affiliates, sales representatives, accountants, billing and col- <br /> lection services, programming suppliers, and authorized representatives of the franchising authority. <br /> Personal information will be maintained by the cable system only so long as it is necessary for <br /> legitimate cable business purposes. Thereafter, such information will be destroyed unless there are <br /> pending requests for access to such information either by the affected subscriber or pursuant to a <br /> court order. <br /> Subscribers may review any such personal information which pertains to them and may request <br /> correction of any errors in such information. If you should desire to review such information at our local <br /> business office, please notify us in writing and an appointment will be arranged promplty for such in- <br /> spection during regular business hours. <br /> U.A.
