4 I, <br /> (c) An employee who becflmes ill, injured or disabled off- <br /> the-job shall cause notice tl�reof to be given to his supervisor not <br /> lateY than the end of the first hour of his next scheduled duty day. <br /> In the absence of evidence of exterYuating circurnstances, an employee <br /> whose supervisor is not so notified within 2 duty days from the day <br /> on which the employee was first scheduled to report shall be eonsider- <br /> ed to have resigned from the classified service. <br /> (d) A physician's statement may be required before sick <br /> leave pay is paid. However, in all cases �iere four consecutive duty <br /> days are missed by an employee due to illness, injury or disability, <br /> a physician's statement will be re quired before sick leave is paid. <br /> Such statements shall set forth reasons for the em�loyee 's inability <br /> to perform his duties and shall become a part og the employee 's per- <br /> sonnel record. Physician's statements will be used in determining <br /> from time to time w�ether or not an enployee is able to continue the <br /> performance of his duties. <br /> (e) If an employee is on sick leave and it is determined <br /> by a physician that said employee will be unable to return t o work <br /> within an amount of time equivalent to his remaining unused sickleave, <br /> said employee must apply for disability benefits if he is eligible for <br /> such benefits and if granted, sick leave pay�ent will be stopped. If <br /> disability is awarded by the employee 's retirement fund he may choose <br /> to be paid for his unused sick leave at one-half his normal daily rate <br /> of pay for each 1.4 unused sick leave days or he nay Yetain his remain- <br /> ing sick leave and be credited therewith if and when he returns to duty. <br /> (f) An employee fou�nd engaged in other employment comparable <br /> to his regular duties �h ile on sick leave will be ordered back to work and <br /> if he refuses his sick leave payment will be stopped. <br /> (g) Upon retirement from the classified fixe fighting service, <br /> an employee shall be paid $2.00 for each day of his unused accumulated <br /> sick leave. <br /> (h) Other than regular shi.ft employees in the classified fire <br /> fighting service shall accxue one duty day with pay sick leave for each <br /> month of continuous seYvice uninterrupted by resignation or discharge up <br /> to a maximum accumulation of 135 days. One day of accumulated sick leave <br /> credit with pay shall be deducted fro�a sick leave accumulated for each <br /> duty day not served due to illness. � <br />
