Said person expressly agrees to, if requested by the City, within 14 days of such request submit an Employer Report in the form prescribed and furnished by the City, and to otherwise comply with the provisions of this ordinance. Section 6. The City Manager is authoriaed and directed to cause to be prepared and furnished forms for said Employer Report ap- proximating as near as may be considering the differences in applica- bility similar forms from time to time in use by the Illinois Fair Em- ployment Practices Commission. Section 7. Requests by the City for Ernployer Reports shall be made by the City Manager or upon his authorization. Section 8. Compliance reviews of an Employer Report may be authorized by the City Manager to be made by the Director of Community ' Relations, The Director may request further relevant information from any person who has submitted an Employer Report as requested so long as such information is reasonably necessary for a proper determination. Section 9. Tf the Director determines that a person is in com- pliance with the Fair Employment Practices Act and the Rules and Regula- tions thereunder, said person, the City Manager and other concerned City officers shall be so notified.
