Said person expressly agrees to, if requested by <br /> the City, within 14 days of such request submit an Employer Report <br /> in the form prescribed and furnished by the City, and to otherwise <br /> comply with the provisions of this ordinance. <br /> Section 6. The City Manager is authoriaed and directed to <br /> cause to be prepared and furnished forms for said Employer Report ap- <br /> proximating as near as may be considering the differences in applica- <br /> bility similar forms from time to time in use by the Illinois Fair Em- <br /> ployment Practices Commission. <br /> Section 7. Requests by the City for Ernployer Reports shall <br /> be made by the City Manager or upon his authorization. <br /> Section 8. Compliance reviews of an Employer Report may be <br /> authorized by the City Manager to be made by the Director of Community <br />' Relations, The Director may request further relevant information from <br /> any person who has submitted an Employer Report as requested so long as <br /> such information is reasonably necessary for a proper determination. <br /> Section 9. Tf the Director determines that a person is in com- <br /> pliance with the Fair Employment Practices Act and the Rules and Regula- <br /> tions thereunder, said person, the City Manager and other concerned City <br /> officers shall be so notified.
