Applications for a license to operate an ambulance or invalid <br /> coach service and renewals thereof shall be made upon forms prescribed by <br /> the City Manager. The applications shall containt <br /> a. The nanie and address of the applicant; <br /> b. The trade or other fictitious name, if any, under a�ich the <br /> applicant does business or praposes to do business; I <br /> c. The location and description of the place or places fxom which <br /> the a�abulance or iavalid coach service operates or is intended to be <br /> o�erated; <br /> d. A description of each ambulance or invalid coach which the <br /> applicant operates or intends to operate within the license year, includ- <br /> ing the make, model, year of manufactnre, serial number, the length of <br /> time the ambulance or invalid coach has been in use, the color scheme, <br /> insignia, name, n�nogram or other distinguishing characteristics to be <br /> used to designate applicant's ambulance or i.nvalid coach, and any unusual <br /> modification of oxiginal chassis to meet basic requirements for an ambu- <br /> lance or invalid coach; <br /> e. 9ach other infor�wtion as the City Manager may reasonably re- <br /> quire in the aci�ainistration of the provisions of this Chapter. <br /> 4. All applications for Iicenges or renewals thereaf shall be <br /> accompanied by an application fee of $100.00 plus $10.00 for each ambu- <br /> lance to be aperated during the license year �ich license year sha11 com- <br /> mence on October 1 and end the following September 30. FoY an application <br /> to operate an invalid coach service, the application shall be accompanied <br /> by a fee of $100.00 plus $10.00 per coach stated on the application. Al1 <br />
