' � , <br /> , '� ` 2 <br /> barriers and warning signals around such excavations or openings, the re- <br /> storation of or failure to restore the surface of any street, sidewalk, <br /> alle y or public way and conditioned further that such restoration of said <br /> surfaces, including the filling of any excavation, be done to the satis- <br /> faction of the Director of Public Works. <br /> Section 7. In addition to the required bond as provided aforesaid, <br /> such person, firm or corporation shall furnish to and keeQ on file with <br /> the City Clerk a certificate of insurance evidencing that there is in full <br /> force and effect at all times public liability insurance including the City <br /> of Decatur as a named insured, insuring said person, firm or corporation <br /> and said City against liability connected with such ru�rk with policy limits <br /> of not less than $50,000.00 for property damage and $100,000.00 for pe rson- <br /> al injury or death for any one person and $300,000.00 for personal injury <br /> or death for any number of persons in any one occurrence . <br /> Section 8. Before commencing any plumbing work a permit shall be ob- <br /> tained from the Administrative Authority upon application the form of whic h <br /> shall be furnished and prescribed by said Administrative Authority and <br /> which shall show the name of the owner of premises wherein work is to be <br /> done, the address of said premises, the person or firm who is to perform <br /> the work, the nature of the work and such other reasonable information as <br /> said Administrative Authority shall require to enable it to carry out the <br /> duties thereof in accordance with said code and this ordinance. <br /> Sect ion 9. No pe rmit shall be issued for wn rk to be done by anyone <br /> other than a certified Iicensed plumbing contractor except for wc�rk to be <br /> done in an o wner occupied single family residence and then only to such <br /> owner after he shall have su bmitted a sketch or plans to the Inspection <br /> Division showing the proposed work and indicating the type of material to <br /> be used. <br /> Section 10. No permit shall be issued for work to be done by a per- <br /> son, firm or corporation that has failed to correct other defective or <br /> faulty work as directed by the Inspection Division until such defective or <br /> faulty work has been corrected and brought into compliance with the provi- <br /> sions hereof. <br /> Section 11. Except as may be other wise directed by the Inspection <br /> Division, any permit under which work has not commenced and been reason- <br /> ably prosecuted to completion shall expire six months from the date of <br /> issuance thereof. <br /> Section 12. No permit shall be issued until the fee for the same, <br /> determined as follows, shall have been paid in full : <br /> For each required inspection including interim, rough- <br /> in and final inspections, $4.00; <br /> For traps, fixtures , appliances or appurtenances (in- <br /> cluding attachments of heating, cooling, air condition- <br /> ing, or refrigeration systems) attached to drainage or <br />
