Caswell City of Decatur ' Attachments: Pre-Bid Meeting Outline, Meeting Attendance Record Sheet ' The following is a summary of the items discussed and items to be coordinated for the project. 1. A Pre-Bid meeting with contractors to discuss the general project scope, bidding instructions, ' and requirements. Attendance and sign in at this meeting is required to bid on the project. 2. The general project overview was discussed outlining the combined sewer separation in the Nelson Park Neighborhood to outlet into Lake Decatur. ' 3. Discussed the Minority Participation Requirements of the City of Decatur including required forms, project goals, and good faith efforts described in Item III of the Pre-Bid Meeting Outline. The City requires direct contact for a demonstration of good faith efforts and looks for 2-3 direct ' contacts for each bidding opportunity unless the goal is met. Provide documentation of all contacts, who was contacted, how they were contacted, and the results of the contact. 4. Discussed the TEPA Disadvantaged Business Requirements outlined in the Project Manual ' pages 90-105. 5. Discussed the IEPA Loan Project Requirements including the use of American Iron and Steel Products, Davis-Bacon Wage Rates, and requirements for Affirmative Action to Ensure Equal ' Employment Opportunity. 6. Discussed the required Storm Water Pollution Prevention Plan. The City questioned whether we needed to add SWPPP plans to the report written by BWC. The City also requested a ' special provision be written for the SWPPP and to note that the price of the SWPPP would be included in the associated line items. The SWPPP will be added as the first Addenda to the bid documents. The City discussed filling out the notice of intent and acquiring the land ' disturbance permit.
