LETTER OF AGREEMENT <br /> OCCUPATIONAL HEALTH SERVICES <br /> This LETTER OF AGREEMENT ("Agreement") is hereby entered into and effective <br /> this L5� day of , 2017 ("Effective Date"), by and between HSHS MEDICAL <br /> GROUP, INC., an Illin is not-for-profit corporation ("Group") and CITY OF DECATUR, <br /> ILLINOIS ("Client") (individually, "Party," or collectively,the"Parties"). <br /> WITNESSETH <br /> WHEREAS, Group operates a clinic that provides quality occupational health services at <br /> HSHS Medical Group, St. Mary's Hospital, Decatur, Illinois; and <br /> WHEREAS, Client has determined it has a legitimate and demonstrable need for certain <br /> occupational health services for Client employees("Employees"); and <br /> WHEREAS, Group desires to enter into an Agreement with Client to perform drug screens, <br /> physicals, breath alcohol testing, audiograms, respirator fit testing, respirator clearance exams, case <br /> management, functional capacity exams and other occupational health services ("Occupational <br /> Medicine Services")to Employees as requested by Client. <br /> NOW, THEREFORE in consideration of the mutual covenants contained herein, the <br /> sufficiency of which is hereby acknowledged,the Parties hereby agree as follows: <br /> 1. OBLIGATIONS OF GROUP. <br /> 1.1. Assist Client in establishing physical examinations and drug testing specific for the <br /> occupation and in accordance with Occupational Safety and Health Administration <br /> ("OSHA") and Department of Transportation ("DOT")requirements. <br /> 1.2. Facilitate physical examinations, laboratory work and paperwork required for physical <br /> examinations of Employees. <br /> 1.3. Process, distribute and maintain all paperwork and reports associated with Employee <br /> physical examinations and completed drug tests within two (2)business days. <br /> 1.4. Refer Employees to their primary physicians regarding non-occupational illnesses, injury, <br /> or disease. <br /> 1.5. Assist in coordinating medical care and rehabilitation of injured Employees, and payment <br /> for services rendered. <br /> 1.6. Provide case management of worker's compensation injuries and notify the designated <br /> contact at Client of the Employee's current work status (i.e., qualified, non-qualified, or <br /> return to work status). <br /> 1.7. Provide mobile unit as requested to perform occupational medicine services at various work <br /> sites. Mobile unit visits will be scheduled at least ninety (90) days in advance to ensure <br /> availability. Group will schedule before ninety(90)days if openings are available. <br /> 1.8. Ensure all testing facilities, including mobile unit are clean, sanitary and well stocked with <br /> necessary sanitary items and supplies. <br /> 1.9. Conduct Employee examinations and testing within three (3)business days of request. <br /> 1.10. The average wait time, as assessed quarterly, shall not exceed seventy-five minutes, <br /> for on-time appointments. <br /> Page 1 of 6 <br />
