Anderson Plaza <br /> City of Decatur, IL 62523 <br /> Dear Mr. Zientara, <br /> Pursuant to our discussion, please consider this document as an extension of our original Letter <br /> of Understanding dated April 13, 2015 for the services provided to the City of Decatur by JL <br /> Hubbard Insurance and Bonds Agency. Our services will include but not be limited to the <br /> following as per the Request for Proposal "Scope of Services": <br /> ❖ Initial meeting to discuss the goals of the City of Decatur and philosophy surrounding your <br /> employee benefits <br /> ❖ Review of current benefit structure <br /> ❖ Collection and review of paid claim data <br /> ❖ Compile and present a full claims analysis to the City of Decatur. This includes, but is not <br /> limited to the following information: <br /> ➢ Cost per employee with comparisons to national data; <br /> ➢ Health care utilization information (paid claim dollars by inpatient services, outpatient <br /> services, prescription drugs, physician office visits, routine services, chiropractic <br /> services, therapies, etc); <br /> ➢ Claims by diagnosis (categories derived by ICD-9 codes to target illnesses within your <br /> member population); <br /> ➢ Eligibility demographics; and <br /> ➢ Prescription drug analysis. <br /> ❖ Analyze current plan of benefits for recommendation of changes relative to cost <br /> containment, industry norms, wellness initiatives and areas of importance to the City of <br /> Decatur <br /> •'• Ongoing administrative functions; <br /> • ➢ Quarterly reporting with claims analysis to identify key issues and trends <br /> ➢ Employee claims issue resolution as needed <br /> ➢ Employee education meetings as needed <br /> ❖ Annual and Periodic Reporting <br /> 1 <br />
