E. EXTR.A WQR.T{.�ND CH.AATGE ORDERS. The Prafessianal Service Provider shall only <br /> perfarm the work authorized by this cantract and defined in the Scope of Work(attached hereto, <br /> marked Ex.hibit A,incorporated by reference herein and made a part af this Agreement). Should the <br /> size or compiexity of the project exceed the amaunt of work contemptated by this contract ar defined <br /> in t�e Scope of Work,the Prafessianal Service Provider shall obtain written authorization in the farm <br /> of a Change{}rder fr4nn the City's Representative,to perform extra wark bafore such work is actualiy <br /> performed.A Change{arder form is included in this Agreement as Ext�ibit D.The cost to perform any <br /> work prior to written authorization shall be paid exclusively by the Professionat Service Pravider and <br /> shalt nat be reimbursed by the City. <br /> The Professional Service Provider expressly acknowledges, recogniz8s ar►d agrees that the only <br /> authority to approve change arders to this Agreernent or the Scope or Services or the cost(s)Yherein is <br /> with the City Council of the City. <br /> SECTI4N II. BASIC SERVICES <br /> The Professional Serviee Provider shall: <br /> A. SC�PE 4F WQRK. Professianal Service Provider shall provide all services as described in the <br /> Scope c�f Work, E�ibit A. <br /> B. CITY'S REQUIREMEIVTS.Revisw available data and cansult with the City's Representative to <br /> clarify and define the City's requirements for the Praject. <br /> C. CtaMPLETION TIME. The Study Report shall be completed,submitted and accepted by the City's <br /> Representative within the time periad set forth in Ex.hibit C,Project Tirneline. <br /> D. MIN4RITY PARTICIPATION GOALS. The�ity Code requiremen#s for Minority Partzcipation <br /> Goals for Public Works Contracts are in e#�ec#for all work itams#hat require the payment of <br /> Prevailing Wages. The payment of Prevailing Wages is required for aEl construction or demolition of <br /> public works. Typical activities requiring Prevailing Wages are manhole rehabilitatic�n,point repairs, <br /> sewer cleaning and ather construction related wc�rk. The minarity participation gaals are as follows: <br /> 1. Ten percent{I 0%} of the total doliar amaunt of the contract should be perforrned by <br /> minority business enterprises; and, <br /> 2. Eighteen percent(18°l0}af the#atal hours worked should be performed by minority <br /> warkers. <br /> The Professional Service Provider shall pravide evidence of ineeting the City's minority <br /> participatian goals prior to all payments made far Prevailing Wage warl�completed or <br /> provide the City of Decatur documentation on their good faith efforts to comply wzth the <br /> participation goals.
