The rounded three-year average value for each <br /> criterion will then be assigned"points"as follows:no(0)points for an average value of 1 to 9,one <br /> (1) point for an average value of 10 to 11, and three (3) points for an average value of 12 to 15. <br /> The points earned for the 5 criteria will then be totaled, and this sum, ranging from a minimum of <br /> zero (0) to a maximum of fifteen (15) points, shall constitute the employee's Performance <br /> Evaluation score. <br /> Commendations/Reprimands Scoring: Shift reprimands and commendations shall only be <br /> considered if issued within the twelve (12)month period ending thirty (30) calendar days prior to <br /> the posting of the position. All other commendations and reprimands shall only be considered if <br /> issued within the forty-eight (48)month period ending thirty (30) days prior to the posting of the <br /> position. <br /> Shift/Division Commendation 0.5 points <br /> Department Letter of Commendation 1.0 points <br /> Community Service Award 1.5 points <br /> Award of Valor 2.0 points <br /> Life Saving Award 2.0 points <br /> Shift/Division Letter of Reprimand minus 0.5 points <br /> Department Letter of Reprimand minus 1.0 points <br /> Any Suspension of 1 to 3 days minus 1.5 points <br /> Any Suspension of four or more days minus 2.0 points <br /> (e). Seniority Points <br /> Seniority points shall be calculated as of the date of the application deadline. <br /> Military Credit <br /> Persons who have been members of the armed forces of the United States or who, while <br /> citizens of the United States, were members of the armed forces of allies of the United States in <br /> time of hostilities with a foreign country shall receive preference for promotion to positions in the <br /> classified service in the manner provided in 65 ILCS 5/10-1-16,and as such statutory provision is <br /> hereafter amended or re-codified. Such preference shall be in addition that otherwise provided <br /> therein and not in lieu thereof. <br /> For purposes of this section, "time of hostilities with a foreign country" shall mean any <br /> period of time in the past,present,or future during which a declaration of war by the United States <br /> Congress has been or is in effect or during which an emergency condition has been or is in effect <br /> that is recognized by the issuance of a Presidential proclamation or a Presidential executive order <br /> and in which the armed forces expeditionary medal or other campaign service medals are awarded <br /> according to Presidential order. <br /> A person taking the promotional examination who qualifies under applicable provisions of <br /> the statutes of the State of Illinois for a military preference, in order to receive such preference, <br /> shall request in writing that he or she be granted such preference and shall at the same time submit <br /> 13 <br />
