EXHIBIT A-1 <br /> CITY OF DECATUR - PUBLIC WORKS DEPARTMENT <br /> SANITARY SEWER SYSTEM REHABILITATION FOR <br /> LOST BRIDGE NORTH (TA_3N) & FLORIAN (TA_6) BASINS <br /> SCOPE OF DESIGN PHASE SERVICES <br /> 3. Prepare repair and rehabilitation plan sheets to support the bidding documents. <br /> Profile sheets not included. <br /> 4. Prepare technical specifications. <br /> 5. Calculate plan quantities and prepare opinions of probable construction cost to <br /> compare to the project budgets. <br /> Task 3: Sewer Lower Lateral Rehabilitation (from the sewer main) <br /> 1. Procure and coordinate full length sewer lateral inspection services from <br /> preferred CCTV contractor(s) to inspect approximately 1,010 sewer laterals in <br /> Lost Bridge North and Florian basins. COD agrees to clean the sewer mains prior <br /> to lateral inspections. Includes CMT oversight in the field for 2 days/week for 6 <br /> months. <br /> 2. Review prior CCTV inspection and manhole inspection reports and identify <br /> protruding laterals and buried manholes that would hinder lateral inspections. <br /> Develop a list of protruding laterals and buried manholes and forward to COD for <br /> mitigation. <br /> 3. Review specific sewer lower lateral repair and rehabilitation options and vendors <br /> with City staff to determine desired approaches. <br /> 4. Review sewer lateral inspection video and reports (as received monthly) to <br /> determine the laterals requiring repair and/or rehabilitation. <br /> 5. Sewer lateral inspections will be performed to reveal the condition of the sewer <br /> laterals, which prior to these inspections has been unknown. An assessment of <br /> the condition for all sewer laterals inspected will be performed to determine an <br /> appropriate length of rehabilitation needed for each lateral. The cost of <br /> rehabilitating the laterals will be considered in this assessment. The length of <br /> lateral rehabilitation required for LBN and Florian basins may be different. <br /> 6. Prepare repair and rehabilitation plan sheets to support the bidding documents. <br /> 7. Prepare technical specifications. Specifications will direct the winning bid <br /> Contractor their responsibility includes obtaining right-of-entry prior to <br /> rehabilitating the sewer lateral. <br /> 8. Calculate plan quantities and prepare an opinion of probable construction cost to <br /> compare to the project budget. <br /> 9. Coordinate and manage weekly progress meetings with CCTV contractor <br /> performing sewer lateral inspections. <br /> Task 4 — Prepare Bidding and Contracting Documents <br /> 1.
