Appendix A <br /> Prior Council Action <br /> • September 6, 2005 - Council approved Resolution 2005-116 authorizing an agreement with <br /> BWC, in the amount of$35,200, to provide professional design services to rehabilitate the <br /> Garfield Avenue Bridge. <br /> • December 19, 2005 - Council approved Resolution 2005-189 authorizing an additional$7,500 <br /> to the agreement with BWC, for a total not to exceed $42,700, in order to provide structural <br /> analysis and design of the proposed support system for the Garfield Avenue Bridge. <br /> • June 16, 2008- Council approved Resolution 2008-97 authorizing a$6100 agreement with <br /> BWC to provide engineering services for an in depth inspection of the Garfield Avenue Bridge. <br /> This inspection noted further deterioration and recommended a more immediate repair project. <br /> IDOT has ordered that the bridge be signed to prohibit overweight permit loads. <br /> • August 4, 2008- staff updated Council with a"Green Sheet Memo"on the current weight limits <br /> placed on the Garfield Avenue Bridge and expected time frame for further inspections and <br /> construction plans. <br /> • August 18, 2008- Council approved Resolution 2008-125 authorizing an agreement with BWC <br /> to provide engineering services to update the plans and specifications for the rehabilitation work <br /> on the Garfield Avenue Bridge. The cost of this work was not to exceed $15,000. <br /> • October 20, 2008 - Council approved Resolution R2008-173 authorizing an additional$24,000 <br /> to the agreement with BWC, for a total not to exceed $39,000 for engin$ering services to prepare <br /> and update rehabilitation plans and project manual for the Garfield Avenue Bridge. <br /> • April 20, 2009—Council approved Resolution R2009-49 authorizing an Agreement with BWC <br /> for Engineering Construction Services for a total not to exceed $15,000 for the Garfield Avenue <br /> Bridge; Resolution R2009-47 to appropriate funds for an Agreement with the State of Illinois <br /> and R2009-48 approving the use of Motor Fuel Tax Funds (MFT) for the improvement of the <br /> Garfield Avenue Bridge. <br /> • July 6, 2009—Council approved Resolution R2009-114 authorizing Lobal Agency Amendment <br /> 1 for Federal Participation to utilize a maximum of$792,000 in Federal,ARRA funds and <br /> $374,473 in City matching funds and Resolution R2009-115 for an additional appropriation of <br /> $200,000 for a total of$380,000 in MFT funds to pay the City's estimated portion of the <br /> Garfield Avenue Bridge. <br /> • November 2, 2009—Council approved Resolution R2009-212 authorizing an Amendment to the <br /> Engineering Services Agreement for an amount not to exceed $8,000 with Blank, Wesselink, <br /> Cook and Associates(BWC) regarding the improvement of the Garfield Avenue Bridge over the <br /> CNIC Rail Road. This is in addition to a previously approved amount Of$15,000, bringing the <br /> total to a not to exceed amount of$23,000 for construction observation assistance. <br /> Page 13 <br />
