ATTACHMENT B <br /> FOR <br /> TASK ORDER NO. 16 <br /> FOR <br /> CONTINUING AGREEMENT FOR ENGINEERING SERVICES <br /> Owner: City of Decatur, Illinois <br /> Engineer: Black &Veatch Corporation <br /> Project: Potential Well Site Exploration <br /> Date: November 28, 2011 <br /> COMPENSATION <br /> For the services described in Attachment A of this Task Order, the Owner agrees to pay <br /> the Engineer as follows. <br /> A. For Phase I Investigation, a fixed price of Seventy thousand and fifty dollars <br /> ($70,050.00). <br /> B. For Phase II Investigation with only one (1) aquifer test, if authorized by Owner, a <br /> fixed price of One hundred and fifty-four thousand, eight hundred dollars <br /> ($154,800.00). <br /> C. For Phase II Investigation with two (2) aquifer tests, if authorized by Owner, a fixed <br /> price of Two hundred sixty-five thousand, seven hundred and fifty dollars <br /> ($265,750.00). <br /> D. For extending test production wells in Phase II to a depth greater than 80 feet, a unit <br /> price of Seventy-nine dollars ($79.00) per foot of additional depth beyond 80 feet. <br /> E. For abandonment and removal of wells, if authorized by Owner, a fixed price of <br /> One thousand dollars ($1,000.00) for each observation well, and a fixed price of <br /> Twelve hundred and fifty dollars ($1,250.00) for each test production well. <br /> F. For Groundwater Modeling, if authorized by Owner, a fixed price of Thirty-one <br /> thousand and six hundred dollars ($31,600.00). <br /> G. Monthly payments shall be made to the Engineer by the Owner based on the <br /> Engineer's statement. <br /> H. The entire amount of each statement shall be due and payable upon receipt by the <br /> Owner.
