Change Order 1 <br /> Street Improvements: South Main and South Franklin Streets <br /> City Project Number: 2010-13 <br /> Page 2 of 2 <br /> Plan revisions— Main Street (Cleveland Avenue to Decatur Street) —Change Pavement <br /> Design — Utilize Channelizing Devices or Detour Route <br /> This work consists of the modifications to the current construction plans, estimates and <br /> specifications for the changes requested by the City in an effort to reduce the cost of <br /> construction. The pavement remove and replace section will be changed to full-depth hot- <br /> mix asphalt surface removal (approximately 6 inches) and overlay with approximately 6 <br /> inches of hot-mix asphalt. To determine the existing pavement composition time will be <br /> spent investigating the pavement thicknesses and conditions from STA 676+00 to STA <br /> 684+00. The remainder of Main Street will be milled and overlaid with 3 inches of Hot-Mix <br /> Asphalt. This section of Main Street will be constructed in stages utilizing standard traffic <br /> control devices (no TCB will be required) or close this section of Main Street and provide <br /> a detour route (to be approved by IDOT Detour Committee). <br /> 168 hours and $19,669 <br /> Striping Plan revisions —Main Street and Franklin Street <br /> This work consists of the modifications to the current pavement marking plans, quantities, <br /> schedules and estimates for the changes requested by the City to provide a <br /> pedestrian/bike lane along the edge of Main and Franklin Streets. <br /> 21 hours and $2,509 <br /> Public Information Meeting — Coordination <br /> This work consists of preparing for and attending a public informational meeting (by <br /> Others) regarding the reduction of the driving lane widths along Main and Franklin <br /> Streets. <br /> 4 hours and $450 <br /> Our proposal is for a total not-to-exceed amount of$29,657 which will complete the <br /> construction plans for this project. <br /> Please contact me with any questions you may have concerning this proposal. <br /> Sincerely, <br /> HOMER L.
