B. Levels of Accomalishment-Performance Goals <br /> In addition to the normal administrative services required as part of this Agreement, <br /> the Subrecipient agrees to provide the following levels of program services with a <br /> completion date as follows: <br /> Units er Month Total Units Com letion Date <br /> Activity-Serve 100 Decatur residents Thursday,April 23, 2010 <br /> C. Staffin� <br /> Project Read Coordinator- 1 <br /> Literacy Specialist-5 (three full-time and two part-time) <br /> Dove Finance Director- 1 (2.5%of time devoted to Project Read) <br /> D. Performance Monitorin <br /> The City will monitor the performance of the Subrecipient against the goals and <br /> performance standards required herein in accordance to the CDBG recipient <br /> monitoring plan. Substandard performance as determined by the City will constitute <br /> noncompliance with this agreement. If action to conect such substandard <br /> performance is not taken by the Subrecipient within a reasonable period of time as <br /> specified by the City and in accordance to the CDBG monitoring Plan, then, after <br /> being notified by the City, contract suspension or termination procedures will be <br /> initiated. <br /> II. TIME OF PERFORMANCE <br /> Services of the Subrecipient shall NOT begin until a Notice to Proceed has been issued by the <br /> Economic and Urban Development Department-Neighborhood Redevelopment Division of the <br /> City. The term of this Agreement and the provisions herein shall end at completion of the fiscal <br /> year, namely April 23, 2010. Program income proceeds (if applicable) shall be accounted for <br /> quarterly, and shall be used only far the purpose outlined in the proposal. All program income <br /> will revert to City if Subrecipient fails to adequately demonstrate that program income was spent <br /> as agreed, or if the Subrecipient ceases to exist. <br /> III. BUDGET <br /> Line Item CDBG Amount Other Funding <br /> Sources <br /> Administration $ 27,350 <br /> Lease $ 3,000 <br /> Caseworkers, Finance $ 5,400 <br /> Specialist, Supportive Service <br /> Director/Homeward Bound <br /> Subrecipient Agreement(Rev. 09/08/09) Page 2 of 18 <br />
