Program Delivery (Exhibit A) <br /> 1. � Financial Assi.stance - Eligible activities include short and medium-term rental <br /> assistance payments, security deposits, utility deposits, utility payments, moving cost <br /> assistance and mote] and hotel vouchers. <br /> 2. � Housing Relocution atid Stabilization Services - Eligible activities include case <br /> management services, outreach to and engagement of eligible program participants, <br /> housing search and placements, legal services to help people stay in their homes, and <br /> credit repair. <br /> 3. � Data Collection: The costs of operating data collection and reporting through the <br /> use of HMIS or a comparable client-level database. Evaluation is also an eligible cost <br /> if the Grantee or Subrecipient is asked by HUD to participate in HUD-sponsored <br /> research and evaluation. <br /> 4. � Administration: Eligible costs include pre-award costs, accounting, reporting, <br /> auditing and staff training. Dove will be responsible for monthly reporting activities <br /> and issuing checks to assist with the activities identified. <br /> B. Levels of Accomplishment - Performance Goals <br /> In addition to the normal services required as part of this Agreement, the Subrecipient <br /> agrees to provide the following levels of program services with a completion date of <br /> September 2011 or before. <br /> C. Staf�in� <br /> Housiiag Program Coordinator-serves as the rapid re-housing coordinator and will be <br /> responsible for overall coordination of the program. Duties will include: conducting <br /> meetings with potential sub-grantees, preparation and submission of reports to the <br /> city, sharing information on the program efforts with the Continuum partners and the <br /> Homeless Advisory Council. <br /> Administrative Specialist-will prepare all intake forms including verification of <br /> income, release of information, confidentiality forms, landlord and rent verification <br /> and other required certification to ensure legitimate use of funds and eligibility of <br /> participants. <br /> Intake Specialist-will assist in the case management assessment, housing <br /> search/placement and other identified case management services. This position will <br /> provide an appropriate goal plan for each participant in the program.
