ADDENDUM TO <br /> INTERGOVER:NMENTAL COOPERATION AGREEMENT <br /> Between City of Decatur, Illinois and <br /> Decatur Civic Center Authority for Inclusion of Authority <br /> in Self-Insurance Program of City, Dated November 7, 1994 <br /> THIS AGREEMENT is an ADDENDUM to an INTERGOVERNMENTAL COOPERATION <br /> AGREEMENT,dated November 7, 1994,between the CITY OF DECATUR,a municipal corporation <br /> organized under the laws of the State of Illinois ("City") and the DECATUR METROPOLITAN <br /> EXPOSITION,AUDITORICTM AND OFFICE BUILDING AUTHORITY,a unit of local government <br /> organized under the laws of the State of Illinois ("Authority"). <br /> WHEREAS, the City and the Authority, on November 7, 1994, entered into an Agreement <br /> entitled "Intergovernmental Cooperation Agreement Between City of Decatur, Illinois, and Decatur <br /> Civic Center Authority for Inclusion of Authority in Self-Insurance Program of City"("Agreement of <br /> November 7, 1994"); and <br /> WHEREAS, the parties hereto desire by this ADDENDUM to the Agreement of November <br /> 7, 1994, to add to the insurance programs of the City of Decatur in which the Authority may <br /> participate; and <br /> WHEREAS,the City has a Self-Insurance program for health care benefits and other programs <br /> in which the Authority may participate and the Authority has indicated a desire to partake under the <br /> Page 1 of 4 <br />
