4. Approval Required: All proposed work must be submitted to the City for written approval prior <br /> to commencing any restoration work(form attached). A detailed summary of the proposed <br /> restoration wark and costs must be submitted to the Department of Engineering and Infrastructure <br /> for approval. The Department will obtain clearance from the Department of Economic and Urban <br /> Development and, when required, the Histarical and Architectural Sites Commission prior to <br /> authorizing the work. If reimbursement costs are $5,000 or greater, the Department must obtain <br /> approval from the City Council. <br /> 5. Permits and Other Requirements: Any permit fees will be waived for this work. Permits may <br /> include a Lane Closure Permit, Permit to Work on Right of Way, and possibly an Electrical <br /> Permit. <br /> a. All work on private property will require a temporary construction easement from the <br /> property owner(s) (a standard form is attached and may be copied as necessary). The <br /> temporary easement form(s)must be included with the application for funding, <br /> b. All work on City right of way must be completed by a contractor that is licensed and <br /> bonded with the City of Decatur, <br /> c. If residents are performing the work, waivers must be obtained from the City and signed <br /> by each participant prior to the work being performed. <br /> 6. Inspection Required: All work will be subject to periodic and final inspection by a <br /> representative of the City. <br /> 7. Reimbursement: Upon completion of the work, the neighborhood should notify City <br /> I Engineering (424-2747) that the work is complete. A City representative will inspect the work ' <br /> and notify the neighborhood of the outcome of the inspection and if any items remain to be i <br /> completed. Upon approval of the work, the neighborhood must submit a Request for <br />' Reimbursement form and attach copies of all receipts for the completed work (or for materials or i <br /> equipment rentals if the work is completed by the neighborhood residents). <br /> a. If a contractor is used, he /she must be licensed and bonded with the City and must pay the I� <br />' applicable prevailing wage for Macon County for the month during which the work is I, <br /> performed. The current wage can be located on the Illinois Department of Labor website I <br /> or by contacting City Engineering at 424-2747. The City must receive a copy of the hourly � <br /> wages paid to each worker along with the receipts for the work. If neighborhood groups <br /> do their own work, prevailing wages do not apply. <br /> b. The City will not reimburse the neighborhood for time or tools purchased for the project. <br /> Equipment rental is reimbursable if provided by a licensed equipment rental agency within <br /> the City limits. <br /> c. City reimbursement of approved expenses will be made within 10 to 15 working days after <br /> the Request for Reimbursement has been submitted and approved. <br />
