� , <br /> ATTACHMENT A <br /> SCOPE OF SERVICES <br /> PROFESSIONAL SERVICES AGREEMENT <br /> Agreement Date: August 2006 <br /> Project: Annual Dam Inspections, Lake Decatur Dam, Dred�g Sediment Storage Facility <br /> Decatur, Illinois <br /> PROJECT DESCRIPTION: <br /> The project includes providing the annual dam inspections for the Lake Decatur Dam in <br /> Decatur, Illinois and for the Dredging Sediment Storage Facility in Oakley Township, Illinois. <br /> SERVICES: <br /> The Scope of Services to be provided is limited to the following: <br /> A registered professional engineer will perform the annual dam inspections for the Lake <br /> Decatur Dam in Decatur, Illinois and for the Dredging Sediment Storage Facility in Oakley <br /> Township, Illinois. Hanson will coordinate the performance of the inspections with the City of <br /> Decatur and with the Illinois Deparhnent of Natural Resources, Office of Water Resources. The <br /> City of Decatur will provide Hanson with copies of the most recent annual inspection reports <br /> for each of these facilities as references and as baseline information in order to more efficiently <br /> perform the requested annual inspections. <br /> The reporting of these inspections will follow the format presented in the Illinois <br /> Department of Natural Resources publication, "Guidelines and Forms for Inspection of Illinois <br /> Dams, November, 1999."
