, <br /> ti► . � <br /> t <br /> ,e� - <br /> F'Y 04/OS CDBG <br /> SUBRECIPIENT AGREEMENT <br /> THIS AGREEMENT, entered this 17th day of May , 2004 by and between the City <br /> of Decatur, Illinois (herein called the "City") and Coalition of Neighborhood Organizations <br /> (herein called the "Subrecipient"). <br /> WHEREAS, the Grantee has applied for and received funds from the United States Government <br /> the U.S. Department of Housing and Urban Development ("HUD") under Title I of the Housing <br /> and Community Development Act of 1974, Public Law 93-383; and <br /> WHEREAS, the City wishes to engage the Subrecipient to assist the City in utilizing such funds <br /> in accordance with HUD guidelines and Federal regulations; <br /> NOW, THEREFORE, it is agreed between the parties hereto that; <br /> I. STATEMENT OF WORK: <br /> A. Activities: <br /> The Subrecipient will be responsible for administering a CDBG Year 2004-2005 <br /> Coalition of Neighborhood Or�anizations/CPTED in a manner satisfactory to the <br /> City and consistent with any standards required as a condition of providing these <br /> funds. Such program will include the following activities eligible under the <br /> Community Development Block Grant Program: <br /> Program Delivery <br /> Activity#1 <br /> Provide grants of up to $750 to low and moderate income homeowners in the city of I <br /> Decatur for security improvements to their homes. = ' <br /> General Administration � <br /> The Program Coordinator will be responsible for administering and overseeing the <br /> - day-to-day operations of the program, including all financial and performance <br /> r-eporting. <br /> B. Levels of Accomplishment- Performance Goals <br /> In addition to the normal administrative services required as part of this Agreement, <br /> the Subrecipient agrees to provide the following levels of program services with a <br /> completion date as follows: <br /> Subrecipient Agreement(Rev.
