ATTACHMENT A FOR TASK ORDER NO. 1 FOR CONTINUING AGREEMENT FOR ENGINEERING SERVICES Owner: City of Decatur, Illinois Engineer: Black &Veatch Corporation Project: South Water Treatment Plant Basin Inspections and Report Date: DESCRIPTION OF SCOPE OF SERVICES The scope of services for Task Order No. 1 shall generally consist of the following: 1. Perform a visual inspection of one primary basin and one secondary basin at the South Water Treatment Plant. The inspection shall be performed after the basins have been dewatered by Owner's staff. The inspection shall consist of visual observation of all of the visible features of the basins including basin equipment, piping, and concrete surfaces. Owner shall supply ladders and other forms of access as Engineer may require to perform a thorough inspection of otherwise inaccessible areas. The inspection shall be performed in a single 8-hour period by a three-person team consisting of two of Engineer's staff plus a representative from the existing basin equipment manufacturer(Ondeo-Degremont). 2. Prepare a written letter report(approximately 10 pages or less)to communicate to Owner a summary of the Engineer's observations during the inspection, the Engineer's recommendations for repairs to the basins and basin equipment, and the Engineer's opinion of probable costs for the recommended repairs. Engineer shall initially submit three copies of a draft letter report for Owner's review. Following receipt of Owner's comments, Engineer shall prepare and submit one copy of Engineer's final letter report. 3. Engage the services of a representative from the existing basin equipment manufacturer (Ondeo-Degremont) to assist with the inspection and report for the existing basin equipment.
