• reporting to the program's Program Effectiveness Assessment Committee(a committee made <br /> up of representatives of the Nationat Partner organizations)of the Partner's accomplishments. <br /> C. Commuaications Coordinator. The Water Suppty Partner agrees to appoint a representative of <br /> the organization (designated on Attachment A, and who may be the same individual as the <br /> Pro ram Coordinator as the Partnershi for Saf <br /> g ) p e Water Communicat�ons Coord�nator. The <br /> Communications Coordinator will direct the Partner's communications effort to ensure(as <br /> deemed appropriate by the Water Suppiier)that the organization is properly recognized for its <br /> participation and accomplishments under the Partnership for Safe Water Program, and in <br /> addition to educate emptoyees and customers about the Program. <br /> D. Partnership for Safe Water Program Requirements. There aze four phases of participation <br /> by Water Supply Partners in the Partnership for Safe Water Progsam. The basic tevel- Partner- <br /> is a Public Water Supply System meeting eligibility requirements and committing(by signing <br /> this Partnership Agreement)to progress to subsequent phases of inembership at least through the <br /> third or self-assessment and conection phase. The three remaining phases-data collection and <br /> analysis, self-assessment and correction,and a third party assessment and implementation- <br /> reflect increasingly higher levels of commitment and accomplishment. The Phase- II data <br /> collection, and the Phase-III self-assessment and correction requirements aze detailed in the <br /> "Partnership for Safe Water: Filtration Process Improvement Self-Assessment Manual" which <br /> accompanies this signature document. The procedures for the Phase- IV third party assessment <br /> and implementation are contained in the guidance documents included in the information packet <br /> supplied to utilities upon application to the process. <br /> E. Reporting. The Water Supply Partner agrees: <br /> • within 180 days of signing this Partnership Agreement,to submit the Baseline Report(as <br /> detailed in the information accompanying this signature document). In general, the Baseline <br /> Report consists of a separate report for each treatment plant of the output from the <br /> Performance Assessment and Major Unit Process Evaluation computer spread sheets <br /> (electronic computer files must be submitted). <br /> • to submit annual reports by June 30 of each year. T'he annual report consists of the same <br /> Performance Assessment spread sheet output(computer files required),as discussed above <br /> for the one year period ending the previous month(May 31), and a nanative report in letter <br /> format outlining the activities undertaken at each treatment ptant or continuing during the <br /> year. <br /> • � to submit a completion report when meeting the requirements for subsequent phases of <br /> membership. The Phase III Self-Assessment Completion Report consists of(1) a cover <br /> letter; (2)a filled in check list of required submittals from the"Partnership for Safe Water: <br /> Filtration Process Improvement Se(f-Assessment Manual"(3)copies of the required <br /> submittals including the Major Unit Process Evaluation spread sheet output; (4)a copy of <br /> the baseline Performance Assessment spreadsheet output, (5)a list of areas identified where <br /> improvements were needed and the action taken on each,(6)the Performance Assessment <br /> spread sheetprintout using the data from the previous year(ending the month before <br /> Partnership for Safe Water Page 4 <br />
