ATTACHMENT III <br /> AGENCY: EASTER SEALS OF CENTRAL ILLINOIS <br /> PROJECT: "Equipment Loan and Home Modification Safety Program" <br /> PROGRAM DESCRIPTION: Due to the consistent number of requests that go unfilled from <br /> low to moderate income residents, Easter Seals is requesting funds needed to continue the home <br /> modification safety program and to allow the availability of durable medical equipment loans on <br /> a short term, urgent need basis. To reach as many in need as possible, a ceiling of$950 would be <br /> allowed for each piece of inedical equipment or to allocate for loan and installation of safety <br /> devices. <br /> FUNDING REQUESTED: $24,134 <br /> FUNDING RECOMMENDATION: 2$ 4.000 <br /> REVISED FUNDING RECOMMENDATION: 1$ 5,000 <br /> RATIONALE: Based on HUD statutory and regulatory requirements this program meets <br /> the national objective for benefit to low/moderate income persons and is <br /> therefore eligible for funding. Local requirements have been meet. The <br /> agency has received funding 3 of the last 4 fiscal years. The funding <br /> recommendation is reduced based on Council's directive. <br /> Page -5- <br />
