It will be designed and structured to conform to current safety and health <br /> policies and management practices,and to eliminate excessive rework of current safety practices. <br /> It will include existing procedures and practices whenever practical and appropriate. Donohue <br /> will prepare a draft manual to be utilized for identification of specific objectives and tasks, and to <br /> identify the organizations or individual positions responsible for each. Donohue will prepare and <br /> submit to the City for review and approval the final manual reflecting all PSM issues addressed <br /> during this project. <br /> Task 3: Employee Participation <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Development and training of the PSM Team; <br /> • Presentation of the PSM responsibilities to PSM Team and other personnel; and <br /> • Development of the documentation required by this element. <br /> Task 4: Process Safety Information <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Identification, inventory,and presentation of requisite process chemistry information; <br /> • Identification, inventory, and presentation of requisite process technology information; <br /> • Identification, inventory, and presentation of requisite process equipment information; and <br /> • Development of the documentation required by this element. <br /> Donohue will develop all block-flow diagrams required by the PSM standard for the processes <br /> and process-equipment configurations utilized at the facility. Donohue will identify the need for <br /> any additional information after the block flow diagrams have been prepared,and the subsequent <br /> equipment manuals have been reviewed for the presence of Process and Instrumentation <br /> Diagrams(P&IDs). <br /> Task 5: Process Hazard Analysis <br /> Donohue will provide services to assist the City in conducting a Process Hazard Analysis(PHA). <br /> The PHA will be conducted utilizing the "What-If Analysis"technique. The purpose of a What-If <br /> Analysis is to identify hazards,hazardous situations, or specific accident events that could <br /> produce an undesirable consequence. An experienced group of facility personnel will identify <br /> possible accident situations,their consequences,and existing safeguards,then suggest alternatives <br /> for risk reduction. <br /> The method involves examination of possible deviations from the design, construction, <br /> modification,and operating intent.It requires a basic understanding of the process to be <br /> evaluated,along with the ability to mentally combine possible deviations from the design intent <br /> that could result in an accident. The What-If Analysis technique will generate a list of questions <br /> and answers about the process. A tabular listing of any hazardous situations,their consequences, <br /> safeguards, and possible options for risk reduction will be prepared.Donohue will: <br /> Donohue&Associates,Inc.
