�- "• Attachment"A" -Scope of Services City of Decatur,uiinois <br /> June, 1999 <br /> • Coordinate development of requisite chemical,technology, and equipment information in <br /> preparation of the PHA; <br /> • Prepare the PHA participants through a review of the PHA techniques, its intent, its <br /> conduct, and its expected outcome; <br /> • Facilitate the conduct of the PHA,utilizing facility personnel, information,and resources, <br /> and the "Supplemental Questions for Hazard Evaluation" contained in"Guidelines for <br /> Hazard Evaluation Procedures" as recommended by the Center for Chemical Process <br /> Safety. (It is anticipated that two to three sessions of no more than a 4-hour duration will <br /> be required.) <br /> • Develop all documentation required for the preparation, conduct, and output of the PHA; <br /> and <br /> • Develop the documentation required by this element. <br /> Task 6: Operating Instructions <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Identification of operating instructions required by the PSM standard; <br /> • Development of operating instructions not present at the City of Decatur South Water <br /> Treatment Plant; <br /> • Presentation of these operating instructions in a format approved by the City of Decatur, <br /> and satisfying the criteria required by the standard; and <br /> • Development of the documentation required by this element. <br /> Task 7: Training <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Development of the training agenda and curriculum for the following PSM elements: <br /> ✓ Process Safety Information; <br /> ✓ Operating Procedures; <br /> ✓ Emergency Procedures;Mechanical Integrity; <br /> ✓ Contractor Procedures; <br /> ✓ PSM Program Overview; and <br /> • Development of the documentation required by this element. <br /> Task 8: Contractors <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Development of"Contractor Information Package,"reflecting the required information <br /> regarding potential hazards related to contractor work, as well as applicable facility safety <br /> rules and emergency response provisions; <br /> • Development of contractor safety performance assessment criteria; and <br /> • Development of the documentation required by this element. <br /> Donohue&Associates,Inc.
