`• '� Attachment"A" -Scope of Services City of Decatur,minois <br /> June, 1999 <br /> Task 9: Pre-Startup Safety Review <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Development of the "Pre-startup Safety Review Checklist;" and ' <br /> • Development of the documentation required by this element. , <br /> Task 10: Mechanical Integrity I'� <br /> Donohue will provide mentoring and coordination services to assist the City in: ', <br /> • Identification of equipment to be addressed under this element; II <br /> • Identification of manufacturer recommendations of maintenance, inspection, and/or <br /> testing actions for such equipment; <br /> • Documentation of current facility maintenance.Inspection,and/or testing actions of such <br /> equipment; <br /> • Presentation in matrix form of equipment; mechanical integrity actions; frequencies of <br /> such actions; facility personnel resource requirements; non-facility resource <br /> requirements; <br /> • Identified parts/material resource requirements. It is anticipated that this document can be <br /> used for budgeting and scheduling purposes,for implementation of the mechanical <br /> integrity element requirements,and as foundation document for future expansion into a <br /> formal maintenance management system; and <br /> • Development of the documentation required by this element. <br /> Task 11: Hot Work Permit <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Review of the existing "Hot Work Permit Program"to identify all PSM requirements, <br /> and to recommend modification as necessary; <br /> • Reflection of the "Hot Work Permit Program" into the PSM Program Manual;and <br /> • Development of the documentation required by this element. <br /> Task 12: Mana ement of Change <br /> g <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> • Development of the Management of Change conceptual process; <br /> • Development of the Management of Change check list to reflect the conceptual process; <br /> and <br /> • Development of the documentation required by this element. <br /> Task 13: Emergency Planning <br /> Donohue will provide mentoring and coordination services to assist the City in: <br /> Donohue&Associates,Inc.
