1. The consultant will deliver thirty(30) copies of the final version of the 2020 Transportation Pdan, along with all maps and graphics to the City of Decatur, which will forward copies to DUATS. 2. The consultant will prepare a 4-16 page summary of the plan for distribution to the media and public. 3. The consultant will present his/her final product to a study session of the Decatur City Council at a time mutually agreeable to both parties. IV. Please be advised that successful candidates will have on file with the State of Illinois an Affirmative Action Plan and, if a riunority business enterprise, a statement of ownership. Ten(10) copies of the response should be forwarded, containing: A. Identification. A title sheet or equivalent which includes the short title for the proposed project; the name and business addresses of the consultant and any subcontractors which he/she proposes to use; and the names, titles, mailing addresses, and phone numbers of the principals and managers involved in the project. B. Study Approach. A brief summary of the approach which the consultant would take in performing the project. C. Qualifications of key persons. The names of the persons who will perform the project, including their academic backgrounds and career experience;their roles in the project; the hourly rates which will be charged for their services; and the total projected costs assigned to each phase of the work. This statement should be accompanied by a description of the experience of each key person in performing related work in the past five (5)years.
