. � �amount of not less than $1,000,000 single limit, $2,000,000 aggregate, for bodily injury, death and <br /> property damage to any one person and subject to the same limitations for any one occurrence. The <br /> comprehensive general liability policy shall include coverages for premises and operations, a blanket <br /> contractual liability coverage which includes the indemnification agreement contained in the next <br /> preceding paragraph, broad form property damage, independent contractors and personal injury <br /> liability coverage. The policy issued for the aforedescribed coverages shall name the City as an <br /> additional insured. Such insurance shall not be cancelled prior to the termination of this Agreement. <br /> 9. Certificate of Insurance. Prior to the commencement of the terms of the Lease, Tommy <br /> House shall file with the City, and at all times thereafter during the term of this Lease keep in full force <br /> and effect, a Certificate of Insurance, in a form acceptable to the City, signed by an authorized agent of <br /> the insurance company or companies writing the insurance described in said Certificate showing <br /> complete coverage of all insurance required in this Lease. Such Certificate shall certify to the <br /> following: Name and address of party insured; name(s) of the insurance compan}-(s); name and <br /> address of authorized agent executing such Certificate; the description of the type of insurance, the <br /> coverages afforded thereunder; the insurance policy numbers; the limits of liability of such policies and <br /> the date of expiration of such policies. Further, the insurance companies shall certify that said policies <br /> shall not be modified, cancelled or terminated until after no less than 30 days v�-ritten notice to the City. <br /> The Certificate shall specifically state that (1) the insurance certified therein contains contractual <br /> liability coverage for the indemnity agreement contained in this agreement, subject to the terms, <br /> conditions and exclusions of the policy, and (2) the City is named as an additional insured on the <br /> comprehensive general liability and property damage insurance described above. The name and <br /> address of the Certificate holder shall be stated as follows: City of Decatur, Illinois, a municipal <br /> corp��ration, #] Gary K. Anderson Plaza, Decatur, Illinois 62523.
