A "client contact", for purposes of information <br /> and assistance, means the receipt of a request and the response <br /> thereto; each request plus response equals one client contact. <br /> Outreach Service - a service whereby the service provider <br /> initiates contact with a senior citizen for the purpose of providing <br /> such citizens with information regaxding various substantive <br /> services available to the citizen. Such contact must be in person, <br /> and one-to-one. Available substantive services are the same as <br /> those included under Information and Assistance. A "client <br /> contact", for the purposes of outreach service, includes any direct <br /> personal contract with a senior citizen, in the home of the senior <br /> citizen or family member thereof, following the initial contact, if <br /> made for the purpose of providing additional information or i <br /> follow-up to information already provided. <br /> Circuit Breaker Program - a state-wide property ta�c relief <br /> program administered by the Illinois Department of Revenue which <br /> issues cash grants to Illinois residents 65 years of age or older who <br /> have household incomes under a certain level. Applications must <br /> be filed with the State on or before December 31. The service <br /> provider would assist such persons with these applications and file <br /> them for the citizen, and provide follow-up services. <br /> Pharmaceutical Assistance Program - a state-wide program <br /> for persons who are eligible for the Circuit Breaker program <br /> whereby the State of Illinois will reimburse qualified persons for <br /> part of the costs for certain medications. The service provider <br /> would assist such persons with the applications for this program <br /> and file them for the citizen, and provide follow-up services. <br /> COLDFACTS Program - a county-wide medical <br /> information program whereby medical information regarding a <br /> senior citizen is recorded on a form, inserted in a bottle and placed <br /> in his or her refrigerator for use by medical emergency response <br /> teams. The service provider would assist in the completion of the <br /> medical information form and verify that it is correctly placed in the <br /> refrigerator, and update regularly. <br /> Carrier Watch Program - a city-wide home check safety <br /> program whereby a senior citizen attaches a sticker to his or her <br /> mailbox, after receiving such sticker through an application process. <br /> Mail which is not retrieved from the mailbox by the next delivery <br /> time will cause the Postal Service to notify the service provider. <br /> The service provider would assist in the completion of the <br />
