The City shall pay the Center Si�y Eight Thousand Dollars ($68,000) for the <br /> performance of those functions and services hereinbelow described. Such sum shall be paid at a <br /> rate not to exceed Five Thousand Seven Hundred Dollars ($5,700) monthly during the term <br /> hereof, and payment shall be subject to full compliance with all performance standards established <br /> herein, subject also to the provisions of paragraph 5. <br /> 3. FUNCTIONS AND SERVICES. A. The Center shall perForm the following <br /> functions and services, and shall meet the minimum standards provided hereby: <br /> Service Service Standard <br /> Information and Assistance 2,200 client contacts annually <br /> Outreach Service 1,500 client contacts annually <br /> Circuit Breaker Program 1,500 applications completed annually <br /> Pharmaceutical Assistance Program 300 applications completed annually <br /> COLDFACTS Program 700 applications completed/updated annually <br /> Carrier Watch Program 300 applications completed/updated annually <br /> All senior citizens assisted for the purpose of this paragraph shall be residents of the City of <br /> Decatur. <br /> B. The Center shall publish and distribute, as described herein, a bi-monthly <br /> newsletter and a directory of services. The bi-monthly newsletter shall discuss and describe the <br /> services available from the Center, provide dates for upcoming events and program deadlines, and <br /> generally keep readers apprised of local matters relevant to senior citizens. The directory of <br /> services shall contain names, addresses, telephone numbers and brief descriptions of agencies and <br />
