ADDENDUM NO. 2 TO THE PLANS, SPECIFICATIONS AND CONTRACT DOCUMENTS ' entitled ' 7T" WARD SEWER REHABILITATION IEPA LOAN NO. L17-5329 CITY PROJECT 2015-07 ' AUTHORIZED FOR BID OCTOBER 29, 2016 AD2-1 NOTICE 1.01 This Addendum No. 1, dated November 28, 2016, to the plans, specifications and contract documents for the above captioned project supersedes all contrary and conflicting information in the documents which are hereby supplemented or revised as follows. eAD2-2 PRE-BID MEETING MINUTES 2.01 Minutes from the pre-bid meeting and the attendance list are attached. AD2-3 MANHOLE PICTURES 3.01 Pictures of the manholes and structures that were taken during the design process are available through Decatur Blue Print. AD24 BID PROPOSAL 4.01 The Bid Proposal form in the Project Manual has been revised. All bids must use the attached Bid Proposal form marked as"ADDENDUM NO. 2". AD2-5 GC-1 CERTIFICATE OF INSURANCE 5.01 Additional Insureds. The Decatur Park District, its officers and employees shall ' also be additional insureds for this contract. Revise the language of the "Certificates"paragraph on page 62 to read as follows: Upon request the contractor shall file with the City certified copies of all insurance policies and all accompanying endorsements. The contractor shall supply certificates of insurance setting forth the coverage, limits, and endorsements before the City will execute the contract. The certificate of insurance shall include both the City of Decatur, its officers and employees; and the engineering firm Bainbridge, Gee, Milanski &Associates, Inc., its officers and employees; and the Decatur Park ' District, its officers and employees as additional insureds.
